Combined score across both validation levels for the selected period. All-level DQA strikes the average of the Sub-District and District scores; the District score itself averages completeness and data-entry, and accuracy is the register-vs-hardcopy match.

Ranking Report

Each indicator shows: current year score, previous year score, and change = current year minus previous year. Green means improved, red means declined.

Note: Change in Contribution is the main ranking and compares every service-area average for the selected current period against the same period in the previous year before calculating the final contribution. Average Score ranking shows only the selected current year: DQA, vaccination average, school children attended, home-visit clients, CWC registrants, and their total arithmetic mean. Number of schools visited is not part of the total average. For 2025, previous-year DQA is fixed at 95% for all CHPS zones.

Public Holidays / Days Off

Record Leave / Excuse

Off Day is a rostered day off for this one member of staff — not a public holiday (those apply to everyone and are set in the Public Holidays card above). Enter the start and end date; a single day means the same date in both. No evidence document is needed, though you may attach a roster if you wish. Off days count as attendance, so they never reduce her percentage.

Not Applicable marks a nurse who does not take attendance on the app at all. No dates and no evidence document are needed — the exemption is open-ended. Her attendance is not calculated: every figure after her CHPS zone and sub-district shows N/A, and she is listed below the ranked nurses. Remove the record from the register if she later starts using the app.

Simple attendance summary only. GPS and photo compliance are intentionally excluded.

Every member of staff appears here — nurses, supervisors and district officers alike. Sign-ins are read from both the field app and the Directorate/facility register. To leave someone out of the scoring, record a Not Applicable entry against her in the leave register above (for staff who do not take attendance on the app, e.g. some Nima Polyclinic staff): she still appears in every report, but her figures read N/A and she is listed below the ranked staff.

Nurse Service Summary

Counts only the service type recorded for each attendance: CWC, Home Visit, School Health or Other Services.

Attendance Register & Summaries

The register lists every member of staff for each date with their rank, sign-in, sign-out and signature. Choose any period — a multi-day register repeats the same daily page for every day in that period, one day after another.

Each download uses the Staff group and Period chosen above. A multi-day period prints one dated register page per working day, one after another.

📊 Performance Presentation

Generate a designed PowerPoint of all services — attendance, nutrition & vaccination, home visits, school health, health promotion, holistic scores, mapping and field photos — for the whole district, a sub-district, or a single CHPS zone, by month, quarter, half-year or year.

Leave & Excuse Duty

Record approved leave, excuse duty and days off, declare public holidays, and download nurse attendance summaries with attendance percentages.

Global Settings

CHPS Zone Establishment

Set once per zone. These auto-fill the header of the CHPS Monthly Report (No. of CHOs / CHMC / CHVs and Total Population). Population defaults from the population master if left blank.

Vaccination-history migration

Set the first date from which a CHPS zone has finished capturing old vaccination histories. From this date, monthly nutrition reports count only antigens newly administered during each visit; earlier green history remains visible and editable.

Health Promotion — data integrity

Scans every saved health promotion activity and fixes entries that break the rules: a missing session type, channel, SBCC material or venue is set to a sensible default, and where the “audience by category” total and the “audience by age & sex” total disagree they are reconciled (the category/audience total is treated as correct and the age & sex breakdown is made to match). Going-forward entries are already blocked at source.

Growth Monitoring — antigen data integrity

Scans saved growth-monitoring records using each child’s recorded age at that visit. Antigens that were not yet due are removed together with their stored or estimated dates, so the child register and reports update consistently. You will see a summary and must confirm before any record is changed.

Data Records Manager

Admin can open the full details of every saved attendance, home visit, growth-monitoring and school-health record, and delete an incorrect record when necessary. Use this when a wrong entry must be removed so the nurse can re-enter it correctly.
